I've found Claude Shannon's original papers to be a good source of inspiration. I'm making this a comment because Shannon did not contribute, or even know about, quantum information theory. You can look in AT&T's archives for his bibliography and try to find the ones you like, or if you're ambitious, you can pick up a copy of his collected papers edited by Wyner and Sloane (1993).

I think this answer is unduly negative with respect to the foundations of quantum mechanics. While physicists "don't know" still is an apt description of the various interpretations of QM, some classes of them i.e. using local hidden variables have been ruled out experimentally beginning with Alain Aspect in the 1980s. The fact that anyone would go to such trouble very much reveals the lie to your assertion that mainstream physicists "don't care".

I'm having some trouble finding any pure states e.g. such as the Z gate on the Bloch sphere representation you've provided. Someone in chat pointed to theorem 2.5 in Nielsen and Chuang providing a reason as to why. Pure states fail both criteria for density operators. Perhaps I've misinterpreted your reasoning and ask that you please clarify.

Niel this is great stuff! Since you asked, I'm teaching myself Q.I.C. from Nielson and Chuang a.k.a. "mike and ike". My hope is that within a year I can get to a point where I can design quantum circuits that are useful.
